Vital Signs Stethoscope
At my new job, they write fake vitals signs into patient records. What do I do?
Background: I am an EMT and recently go hired on to an private ambulance company.
We have to have 2 blood pressure readings for each patient for medicare.
I realize now that most people lie about the vital signs and patient assessment. My coworkers can take a BP without touching a patient. We have a cuff and stethoscope, but they would rather fake it.
I actually do patient assessment and know only takes 1 minute or so to do it right and get a full set of vitals.
I need the money and want to work, but I don't want my conscience or my license to practice to be in jeopardy.
This seems unethical to me.
I have only been at this place a couple of days, minus orientation/training day.
This is very tricky. For one, the safety of the patients that are served is in jeopardy. I am an ER nurse and we depend greatly on the skill and the knowledge of the EMT's and medics that bring the patients to us. I feel that you have a moral obligation to tell your manager or chief. He or she can do nothing without concrete proof. This will make you very unpopular and eventually you will probably quit because your coworkers will make it rough on you. So my advice is this. Tell your chief and be exact on the time, place, situation. They can cross reference the EMS run report with the ER chart. If the BP's are fabricated this will be pretty apparent when you compare the two. If nothing is done about it in about 3 months. Tell your chief that if something is not done about it, you will inform the medical director, the board of directors, and (the kiss of the death for the company is contact Centers for Medicare and Medicaid Services). The chief can't fire you under whistle blower protection.
Remember, it will not be easy. Your life will be really rough, but remember you entered the business not for the money Lord knows that EMT's get paid diddly squat for what they do, you entered to serve the people.

False - it is usually measured at the wrist (radial pulse), in the upper arm in babies (brachial pulse) or in the neck (carotid pulse). You can measure heart rate by listening to the heart, but it's not truly a "pulse" then.
Pulse oximetery measures hemoglobin saturation and the plethysmograph will show you the pulse rate. Respirations are not measured with a pulse ox.
True about BP (although these days, we tend to use oscillometric blood pressure machines)
