Taking blood pressure in fat/obese patients.?
help. why is its easy to take the brachial pulse of thin people unlike when assesing those who have fat arms- I'm having a difficulty listening to their pulse and might include the korotkoff sounds using my stethoscope. If that happens, what should be done to hear it effectively. help.
another, the first time you place the stethoscope should you hear it right away? or some say when you've inflated it's the time you'll hear.

well, lets get things straight...when u put stethoscope on the cubital fossa, u shld not hear anything at all.. this is because the flow of blood thru the brachial artery is not blocked and laminar blood flow is happening...
the right way to take a blood pressure is to put the bell of stethoscope on the cubital fossa, then inflate the cuff of your sphygomanometer until beyond the highest systolic pressure and you still should not hear anything..this is because the pressure is high enuf to completely occlude the brachial artery..
slowly release the pressure on the cuff, and the moment you hear a sound, thats your systolic BP..after that, the korotkoff sounds starts due to turbulent flow of blood...
the moment the sounds disappear, thats your diastolic BP because the blood flow went laminar again...
for fat or obese patients, i think you should try putting part of the bell of the stethoscope inside the bottom part of the cuff, the pressure shld make the sounds clearer..
besides that, do get the right area at the cubital fossa, any slight deviation might be causing you not able to listen to the BP...
