heart murmur question?
This may seem a daft question but somebody I work with has been told that she gets a loud (audible without a stethoscope) after a night out. I suspect due to dehydration. Iam concerned for her how serious or even worrying should this be. She hasnt seen her doctor about and dismissed it when I asked her more
The extra fluid that she's temporarily intaking (eg. beer) is increasing her vascular volume, that is, the amount of fluid within her cardiovascular system (hemodynamically defined as "preload").
This transient increase in preload pressures is making her murmur so loud that its possible to hear without a stethescope. Murmurs are typically graded from 1 through 6. You friend has a grade 6 of 6, that is a murmur so loud that it can be detected with a stethescope that is not even touching her skin (or in this case, without a stethescope at all).
I myself in more than 20 years of practice have only encountered this once (as it turned out, in a 79 year old retired heart surgeon; how's that for irony, eh?).
Whether or not your friend's murmur is clinically significant or is only a conversation starting curiostiy is best decided by her doctor.
