Bell Diaphragm Stethoscope
Which stethoscope is the best one?
not in terms of brands but a stethoscope with two tubes that join into one, or one that has two tubes all the way down to the diaphragm and bell
It is difficult to answer this question because most people want to buy a stethoscope which impresses their friends and are prepared to spend money on it. I mean no disrespect to nurses, but they are not going to be as well trained in interpreting the sounds through a stethoscope as a cardiologist or respiratory physician, and therefore do not need a Littman. For medical students there is no practical difference between one tube or two, but I find two tubes more cumbersome to fold than one. Working in an intensive care environment I would use only the stethoscope at that patient's bedside, to avoid cross-infection, and only very rarely would I get my own expensive stethoscope out.

Stethoscope 101
When listening to blood pressure with a stethoscope, should I use the bell or the diaphragm?
Use the diaphragm - the range of frequency is better for Korotkoff sounds. The bell is usually used for lower pitch sounds such as lung apex breath sounds, carotid bruit and mitral regurgitation.
The bell is sometimes used in paediatrics because of the smaller size. However, in this case you should turn it into a "diaphragm" by pressing harder on the skin. (Or, in some stethoscopes, you can just add a piece of diaphragm onto the bell)
Finally, remember not too press too hard when using the bell, or else you'll have a diaphragm instead!
